Unfortunately in some cases extraction cannot be avoided. The reasons for extracting a tooth can be toothache, poor prognosis of the tooth or orthodontics

However extraction is a routine treatment it is important who takes the tooth out and how the treatment goes. At our clinic oral surgeons extract probematic teeth and we provide maximum care to our patients before and after the procedure.

Before tooth extraction

There are some things to do before extraction in order to avoid later discomfort, complications and make the treatment easier.

The followings are essentials before tooth extraction

  • Thorough brushing and flossing
  • Scale and polish carried out by our dental hygienists

The aim of these dental hygiene instructions and treatments is to achieve as bacteria free enviromnet as possible to avoid bacteria get into the wound.

Apart from brushing, using  a mouthwash is also important before extraction. It is also recommended to avoid smoking and alcohol a few days before and after the treatment.

After extraction

There are some areas that we need to pay attention to after extraction.

Our oral surgeon will do her best in supporting your healing after the treatment but in order to achieve the best results you also need to cooperate in many ways.

What do we need to pay attention to after extraction

  • spitting, rinsing and sucking the wound are not allowed
  • avoid eggs, milk and dairy for 2-3 days
  • avoid smoking and alcohol
  • do not bend or do excercises
  • avoid heavy lifting
  • oozing is normal, in this case sit down and bite on a piece of gauze
  • You can put cold compress on your face in the extraction area. This will reduce post-operative pain  and swelling
  • in 99% of cases it is not necessary, but if indicated you can take painkillers based on your dentist’s instructions

It is important to follow these instructions in order to avoid post-operative complications such as dry socket. If you have strong pain 1-2 days after the extraction, it means that you have dry socket. Dry socket means that you loose the blood clot that was formed after the extraction (because you were rinsing or spitting) and the bone is not covered anymore. Smoking can also cause dry socket. In this case it is important to see your dentist.

The procedure of tooth extraction

  • we thoroughly clen the surrounding area of the tooth to avid infections
  • we gently loosen the tooth and extract it
  • a blood clot will form in the socket where the tooth was
  • the clot protects the bone and stops bacteria get into the wound
  • we place a disinfectant tablet and a special sponge into the socket that stabilises the blood clot
  • in order to support healing and protect the wound we seal it with a few stiches
  • a week after the extraction we remove the stiches and examine the wound
  • the next step is planning the replacement of the extracted tooth if necessary

The whole procedure of extraction is carried out under local anaesthesia and it is absolutely painless

Replacing the extracted tooth

If your tooth was not removed for orthodontic reasons but due to poor prognosis, periodontal disease  or pain, it is recommended to consider replacing it. It is important to replace the gap as soon as possible so the implant/bridge can take over the position and functions of the extracted tooth. The gap can cause serious negative effects in the mouth if it is not replaced in time.

Implants need secure and stabile base. If there is no base, the success will not be guaranteed no matter what implant and what technique we use. We would like your implant to stay stabile and reliable for at leaset 50-60 years.

If there is not enough bone and the sinus is too close, we need to lift it by placing bone grafting material under the sinus. This way we will have enough space to place the implant.

Sinus lift is a safe procedure that provides a secure base for your implant.

What does sinus lift mean?

During the procedure the internal wall of the sinus towards the  missing tooth is lifted and bone grafting material is placed into the cavity that is formed by lifting the sinus. The bone grafting material is usually a mixture of your own bone shavings and a bio bone grafting material.

The procedure is totally painless, fast and safe. 

When the bone grafting material solidifies it will be as hard as natural bone and the implant can be placed in it.

What is the result of bone grafting?

  • adequate base for the adoption of the implant
  • adequate srtrength for a long term tooth replacement that can last for decades
  • the sinus does not injure during the implantation
  • the implant will be stabile and fix because it will be placed in a secure, strong base

Sinus lift can be carried out even in case of multiple tooth replacement. In our climate zone people tend to have thinner bone, therefore we carry out the procedure quite often at our clinic in order to perform perfect implantation.

Microimplants are great help in dental treatments. These 6-7 mm long and 1 mm diameter tiny screws are temporary placed into the bone during treatments and they will get removed once they completed their mission.

Microimplants are mainly used before braces are fitted to replace teeth that have been removed earlier, to correct weakened teeth that are periodontally involved or to correct the slope of the chewing surface.

When size does matter

Microiplants cannot be noticed due to their tiny size but they are still very stabile and provide maximum support for dental treatments and orthodontic procedures.

Discomfort caused by mising teeth can be solved by replacing the missing tooth. Replacements can be: implants or bridges.

What can cause tooth loss and what problems can we avoid if we replace them?

  • missing front teeth can affect the bite
  • we smile less, we can loose our self-confidence
  • missing molar teeth results in chewing difficulties that can lead to digestion problems as well
  • missing back teeth leaves gums unprotected
  • teeth located next to the gaps can move towards each other or drift
  • nodules and inflammation can form under the mucosal tissue causing further tooth loss
  • due to the absence of pressure (no bite) the bone can shrink
  • gums receed and suffer irriversible damages

What are your options for tooth replacement?

If there are teeth on both sides of the missing tooth a bridge can replace tha gap.

Advantages:

  • quick solution and can be carried out in a couple of weeks
  • it matches the shape and shade of your natural teeth
  • it is made of metal free ceramics

Disadvantages:

  • in order to create the two sides of the bridge we need to file both neighbouring teeth down
  • without adequate cleaning food can trap under the bridge causing bad breath and inflammation
  • this solution does not put pressure on the gum, therefore the bone can still shrink causing the gum to receed which increases the gap
  • the lifetime of brides is much shorter than implants’
  • chewing can get uncertain, we can be too concious of chewing when biting harder food
  • bridges can damage and lose their stability

The most stabile solution is implant, which is a screw placed into the bone that forms a secure root for the aesthetic crown.

Advantages:

  • there is no need to file neighbouring teeth for implants
  • they last 40-50 years and often we can experience a 70-80 year lifetime as well
  • we can put as much pressure on it as we do on our own teeth, therefore the bone does not weakens
  • it replaces the gap, therefore neighbouring teth will not move or rotate
  • your gums stay healthy, they will not shrink, gum surgery is not needed
  • chewing is secured for decades, the implant can take harder bites as well
  • the crown and superstructure placed on the implant are stronger and last longer
  • it is made of titanium that the system accepts easily, ossification starts straight after the implant is placed

Disadvantages:

  • the implant process takes longer buti t never lasts for more than 6 months
  • if there is not enough bone for the screw, bone replacement is needed
